Laterality matters. Most foot/ankle codes have right/left/unspecified variants — unspecified codes are a common (and avoidable) denial trigger. Pair diabetes codes correctly: a diabetic foot ulcer is coded with both the diabetes-with-ulcer code and the ulcer code, per the guidelines. Don't code symptoms alone when a definitive diagnosis is documented — "foot pain" (M79.67-) supports medical necessity poorly compared to the underlying condition.
Plantar fasciitis is commonly reported with M72.2 (plantar fascial fibromatosis). Always verify against the current CMS ICD-10-CM tabular list before billing.
M20.10 is hallux valgus of an unspecified foot; add laterality with M20.11 (right) or M20.12 (left) per the tabular list.
E11.621 is Type 2 diabetes mellitus with foot ulcer. The ulcer itself is additionally coded, e.g., from the L97.- non-pressure chronic ulcer series, per coding guidelines.
